Tablet
Treatment of hypothyroidism (underactive thyroid); thyroid hormone replacement after thyroidectomy or radioiodine therapy; TSH suppression in certain thyroid cancers/goitre as advised by a doctor
Levothyroxine is a synthetic form of thyroxine (T4), a natural thyroid hormone. It is converted to the active hormone T3 in tissues and helps restore normal metabolism, energy levels, heart function and growth by normalizing thyroid hormone levels and TSH.
Oral tablet. Take once daily on an empty stomach, preferably in the morning 30–60 minutes before breakfast with water. Take at the same time each day; avoid taking with calcium/iron/antacids/soy/high-fibre foods within 4 hours. Dose is individualized based on TSH/T4; do not stop or change dose without medical advice.
Palpitations, fast heartbeat, nervousness, tremor, headache, insomnia, sweating, heat intolerance, increased appetite, weight loss, diarrhoea, menstrual irregularities
Use only under medical supervision; overdose can cause symptoms of hyperthyroidism and serious heart problems. Use with caution in coronary artery disease, arrhythmias, hypertension, and elderly patients; start low and titrate. Long-term overtreatment may reduce bone density/osteoporosis risk. Not for weight loss/obesity treatment. Monitor TSH and free T4 regularly; dosing requirements may change in pregnancy, with weight changes, or with interacting medicines.
Calcium salts, iron salts, aluminium/magnesium antacids, sucralfate, cholestyramine/colestipol, sevelamer (reduce absorption); proton pump inhibitors and other acid-lowering agents may reduce absorption; rifampicin, carbamazepine, phenytoin, phenobarbital (increase metabolism); sertraline (may increase requirement); amiodarone (affects thyroid function); warfarin (may increase anticoagulant effect); insulin/oral antidiabetics (dose adjustment may be needed); digoxin (effects may change).
