Tolvaptan 30 mg (tablet)
Treatment of hyponatremia (low blood sodium) associated with conditions like heart failure, liver cirrhosis and SIADH (syndrome of inappropriate antidiuretic hormone secretion); used under medical supervision to raise serum sodium.
Tolvaptan is a selective vasopressin V2-receptor antagonist. It blocks the action of antidiuretic hormone in the kidneys, increasing excretion of free water (aquaresis) without significant loss of electrolytes, thereby increasing serum sodium.
Take as prescribed by a doctor, usually once daily. Can be taken with or without food; swallow whole with water. Initiation and dose changes are typically done with close monitoring of sodium and hydration status; maintain adequate access to water and drink when thirsty.
Thirst, dry mouth, increased urination (polyuria), frequent urination, dehydration, dizziness, weakness, constipation, increased blood sodium (hypernatremia)
Use only under specialist supervision with regular monitoring of serum sodium to avoid overly rapid correction (risk of serious neurological injury). Risk of dehydration and low blood pressure—ensure adequate fluid intake. Tolvaptan can cause serious liver injury in some uses; monitor liver function if advised and stop if symptoms like fatigue, loss of appetite, right upper abdominal pain, dark urine or jaundice occur. Avoid in anuria, inability to sense/respond to thirst, hypovolemic hyponatremia, urgent need to raise sodium, or severe liver disease unless specifically directed. Use caution in kidney disease, elderly, and with diuretics.
Strong CYP3A inhibitors (ketoconazole, itraconazole, clarithromycin, ritonavir), CYP3A inducers (rifampicin, phenytoin, carbamazepine, St John’s wort), other diuretics, digoxin (may increase levels), grapefruit juice
