Prescription-only. Risk of pancreatitis (seek care for severe persistent abdominal pain); gallbladder disease (cholelithiasis/cholecystitis) possible. May cause dehydration and worsen kidney function due to GI side effects—maintain fluids. Contraindicated/avoid in patients with personal/family history of medullary thyroid carcinoma or MEN2; discontinue if thyroid mass/hoarseness develops. Monitor for hypoglycaemia when used with insulin or sulfonylureas. Not recommended in severe gastrointestinal disease (e.g., gastroparesis). Stop before major surgery if advised; monitor for diabetic retinopathy worsening in patients with diabetes. Do not use for type 1 diabetes or for diabetic ketoacidosis.
Insulin and sulfonylureas (increased risk of hypoglycaemia), other GLP-1 receptor agonists (avoid duplication), drugs with narrow therapeutic index affected by delayed gastric emptying (monitor as needed), warfarin (monitor INR if clinically indicated), alcohol (may worsen hypoglycaemia risk in diabetics and GI effects).
