Diabetes mellitus (Type 1 and Type 2) requiring insulin therapy; control of blood glucose levels

Biphasic human insulin provides a quick onset from soluble insulin and a longer, intermediate effect from NPH insulin. It replaces/augments endogenous insulin to increase peripheral glucose uptake and reduce hepatic glucose production, thereby lowering blood sugar.

For subcutaneous injection only, as prescribed by the doctor. Usually injected 15–30 minutes before meals; dose and timing are individualized. Roll vial gently to resuspend (do not shake vigorously); use insulin syringes suitable for U-40 strength; rotate injection sites.

Hypoglycaemia, injection site reactions (pain/redness/swelling), lipodystrophy, weight gain, edema, allergic reactions, rash/itching

Risk of severe hypoglycaemia—monitor blood glucose regularly and carry fast-acting sugar. Do not use during episodes of hypoglycaemia. Use with caution in renal/hepatic impairment, elderly, irregular meals, increased exercise, or acute illness; dose adjustments may be needed. Do not mix with other insulins unless advised; ensure correct strength (U-40) to avoid dosing errors. Seek urgent care for severe allergy or recurrent unexplained hypoglycaemia.

Other antidiabetics (additive hypoglycaemia), beta-blockers (mask hypoglycaemia), ACE inhibitors (may increase hypoglycaemia), corticosteroids/thiazide diuretics/thyroid hormones (may increase glucose), alcohol (hypoglycaemia), MAO inhibitors (increase hypoglycaemia), sympathomimetics (increase glucose)

Store refrigerated at 2–8°C; do not freeze. Protect from light. Keep vial in outer carton. Once in use, may be kept at room temperature (below ~25–30°C as per label) for a limited period; discard if frozen, overheated, or if clumping/precipitation persists after resuspension.
