Diabetes mellitus (Type 1 and Type 2) requiring insulin; control of blood glucose levels in patients needing a premixed insulin regimen
This is a premixed human insulin containing 30% short-acting (soluble) insulin for meal-time glucose control and 70% intermediate-acting (NPH/isophane) insulin for basal control. It lowers blood glucose by increasing peripheral glucose uptake and reducing hepatic glucose production.
For subcutaneous injection only as advised by a doctor (commonly 1–2 times daily, about 30 minutes before meals). Use a sterile insulin syringe calibrated for 40 IU/mL (U-40). Roll the vial gently between palms to resuspend (do not shake) until uniformly milky; check for clumps. Rotate injection sites; do not inject into a vein.

Risk of severe hypoglycaemia—monitor blood glucose, carry fast-acting sugar, and avoid skipping meals after dosing. Dose may need adjustment with changes in diet, activity, illness, kidney/liver impairment, or when switching insulin brands/strengths. Do not use if solution contains clumps/particles after proper resuspension or if expired. Use caution when driving/operating machinery if prone to low sugar. Not for IV use; mixing/switching should be done only on medical advice.
Other antidiabetics, ACE inhibitors/ARBs, salicylates and alcohol may increase hypoglycaemic effect; corticosteroids, thiazide/loop diuretics, thyroid hormones, oral contraceptives, sympathomimetics and some antipsychotics may reduce effect. Beta-blockers and clonidine may mask hypoglycaemia symptoms. MAO inhibitors may increase hypoglycaemia risk.
Store refrigerated at 2–8°C; do not freeze. Protect from light. Once in use, keep as per label (often up to ~28 days) and avoid heat; keep vial tightly closed and out of reach of children.
