Erectile dysfunction (impotence) in adult men

Sildenafil is a phosphodiesterase-5 (PDE5) inhibitor. It increases cyclic GMP levels in penile tissue, improving blood flow during sexual stimulation and helping achieve and maintain an erection. Sexual stimulation is required for it to work.

Tablet (oral). Take as directed by a doctor, typically 30–60 minutes before sexual activity (can be taken up to 4 hours before). Do not take more than once in 24 hours. Can be taken with or without food; a heavy/high-fat meal may delay onset.

Headache, flushing, dizziness, nasal congestion, indigestion, nausea, blurred vision, back pain

Not for use with nitrates (e.g., nitroglycerin/isosorbide) due to severe hypotension risk. Use caution in heart disease, recent heart attack/stroke, uncontrolled blood pressure, severe liver/kidney impairment, bleeding disorders, or peptic ulcer. Seek urgent care for chest pain, sudden vision/hearing loss, or an erection lasting >4 hours (priapism). Avoid driving if dizzy or vision is affected. Not recommended in women or children.

Nitrates (contraindicated), riociguat (contraindicated), alpha-blockers and antihypertensives (additive BP lowering), strong CYP3A4 inhibitors (ketoconazole, itraconazole, ritonavir, clarithromycin), CYP3A4 inducers (rifampicin), other PDE5 inhibitors, alcohol (increases hypotension/dizziness), grapefruit juice (may increase levels)
