How It Works
Itraconazole is a triazole antifungal that inhibits fungal cytochrome P450 (14-α-demethylase), blocking ergosterol synthesis. This damages the fungal cell membrane and stops fungal growth and replication.
Directions for Use
Oral capsule. Take exactly as prescribed, preferably after a full meal for better absorption. Swallow whole; do not crush or chew. If you use acid-reducing medicines, take them as advised by your doctor as they may reduce absorption.
Prescription-only antifungal. Can cause serious liver injury—seek help for yellowing of eyes/skin, dark urine, severe fatigue or abdominal pain; liver function monitoring may be needed. May worsen heart failure (negative inotropic effect)—avoid/use with extreme caution in patients with congestive heart failure. Numerous drug interactions via CYP3A4—some combinations are contraindicated due to risk of serious heart rhythm problems. Use caution in kidney/liver disease and in elderly.
Drug Interactions
Contraindicated/major interactions with drugs highly dependent on CYP3A4: simvastatin/lovastatin, certain antiarrhythmics (e.g., quinidine, dofetilide), ergot alkaloids, cisapride, pimozide; increases levels of warfarin, digoxin, cyclosporine/tacrolimus, certain benzodiazepines, calcium channel blockers; reduced absorption with PPIs/H2 blockers/antacids; rifampicin/rifabutin/phenytoin/carbamazepine reduce itraconazole levels.
