How the medical certificate application works
From purpose selection and evidence to clinician review and document delivery.

1. Choose the purpose
Select the certificate that describes the actual request. Fitness for travel and inability to travel are different opinions. Sports and adoption have recipient-specific assessment requirements. State selection is required for Form 1A.
2. Complete the substantial application
Identify the patient and applicant, explain any guardian or caretaker role, name the recipient, describe the condition or assessment purpose, and provide dates and medical history. Answer each screening question yourself; a blank answer is not treated as a normal finding.
3. Provide evidence and select a package
Upload the records and format required for your purpose. Choose a digital, handwritten scan, prescription-assessment or shipping package as available. Review the exact total and the terms applying to your purchase before payment.
4. Clinician assessment
The reviewing clinician may seek clarification, a consultation, measurements, reports or an in-person examination. The requested dates and wording are requests for assessment. They become certificate content only when the clinician supports them.
5. Approval, delivery and verification
After approval, the document is generated from the clinician's recorded findings, reviewed and signed. Digital dispatch, printing and courier transit are separate fulfilment steps. The issued document has its own record; a public specimen is never a substitute. Cancellation and refusal follow the policy saved with the purchase.
