Prepare identity, recipient instructions, medical history and evidence before you apply.

Identity and who is applying
Use the patient's name as it should appear on the document, age or date of birth, contact details and the requested certificate purpose. A parent, guardian or caretaker should identify both themselves and the patient and explain their relationship and authority. Keep identity evidence ready where the application requires it.
Recipient and requested format
Ask the employer, college, event organizer, airline or authority what they need: a digital PDF, original signed paper, a named form, particular assessment fields or an in-person examination. Upload their blank form and instructions. A sample shows layout; it does not establish what a recipient will accept.
History and supporting records
Provide the reason for the request, symptom or illness dates, current treatment, medicines, allergies and relevant prior conditions. Add available consultation notes, prescriptions, discharge summaries or test reports that support the requested period or assessment. Tell the doctor when a measurement was taken and who measured it.
Purpose-specific preparation
For leave or recovery, explain the affected duties and requested dates. For caretaker leave, identify the person receiving care and the assistance needed. For sports, include the named event, activity and organizer's form. For travel, provide the journey and whether you need a fitness opinion or an assessment of inability to travel. Upload and review
Upload through your private application. Required evidence must be provided before the relevant review step; records marked as follow-up can be requested by the clinician. Check names, dates, package, format and declarations before paying. Never send medical records through a public review or analytics form.
