Glimepiride 4 mg + Metformin Hydrochloride (sustained/extended release) (strength unknown)
Type 2 diabetes mellitus (adult) to improve blood sugar control along with diet and exercise
Glimepiride (a sulfonylurea) increases insulin release from the pancreas and helps lower blood glucose. Metformin reduces glucose production in the liver and improves insulin sensitivity, lowering insulin resistance.
Oral tablet; take exactly as prescribed. Usually taken once daily with the first main meal (or as advised). Swallow whole; do not crush/chew if it is SR/ER. Monitor blood glucose regularly.

Risk of hypoglycaemia (higher with missed meals, excess exercise, alcohol, elderly, kidney/liver impairment); carry sugar/glucose. Metformin may rarely cause lactic acidosis—seek urgent care for rapid breathing, severe weakness, abdominal pain, unusual sleepiness. Use with caution/adjust dose in kidney impairment; avoid in severe renal disease. Hold metformin before/after iodinated contrast studies or major surgery as advised by doctor. Monitor vitamin B12 with long-term use. Not for type 1 diabetes or diabetic ketoacidosis.
