Treatment of diabetes mellitus to improve glycaemic control in adults and children where basal (long-acting) insulin is indicated; used in type 1 diabetes (with mealtime insulin) and type 2 diabetes (alone or with oral antidiabetics/other insulins).
Insulin glargine is a long-acting basal insulin analogue. After subcutaneous injection it forms microprecipitates in the tissue, releasing insulin slowly and providing a near-constant 24-hour glucose-lowering effect.
For subcutaneous use only using a compatible insulin pen for 3 mL cartridges. Inject once daily at the same time each day as prescribed; do not mix or dilute with other insulins. Rotate injection sites (abdomen, thigh, upper arm); check insulin and cartridge before use; monitor blood glucose regularly.

Risk of hypoglycaemia—may impair ability to drive/operate machines; adjust dose with changes in diet, activity, illness or stress. Do not use in diabetic ketoacidosis (needs rapid-acting insulin). Not for intravenous use; do not share pens/cartridges. Use caution in renal or hepatic impairment and in elderly; monitor for hypokalaemia. Seek urgent care for severe allergy/angioedema or recurrent severe hypoglycaemia.
Other antidiabetics and alcohol may increase hypoglycaemia risk; corticosteroids, thiazide/loop diuretics, thyroid hormones, sympathomimetics and oral contraceptives may increase glucose (need higher insulin). Beta-blockers, clonidine, guanethidine and reserpine may mask hypoglycaemia symptoms. ACE inhibitors/ARBs, salicylates and MAO inhibitors may enhance insulin effect; pioglitazone with insulin may increase risk of fluid retention/heart failure.
Store unopened cartridges refrigerated at 2–8°C; do not freeze; keep away from the freezer compartment. Protect from light. After first use, keep at room temperature (generally below 25–30°C as per label), avoid heat and sunlight; discard after the in-use period stated on the pack (commonly ~28 days).
