Diabetes mellitus requiring basal insulin: Type 1 diabetes (with short/rapid-acting insulin) and Type 2 diabetes when diet, exercise and/or oral antidiabetics are insufficient
Insulin glargine is a long-acting basal insulin analogue. After subcutaneous injection it forms microprecipitates that slowly release insulin, providing near 24‑hour glucose-lowering with a relatively flat profile.
For subcutaneous use only. Inject once daily at the same time each day into abdomen, thigh or upper arm; rotate injection sites. Use the cartridge only with the compatible reusable pen as instructed in the initiation kit; prime the pen before each use and do not share needles/pen. Can be taken with or without food; dose is individualized by the doctor.
Hypoglycaemia, injection site pain/redness/swelling, lipodystrophy, weight gain, peripheral edema, rash, itching
Risk of hypoglycaemia—monitor blood glucose and carry fast-acting sugar. Do not use for treatment of diabetic ketoacidosis or via IV/insulin pump. Do not mix or dilute insulin glargine with other insulins. Use caution in renal/hepatic impairment, elderly, irregular meals, increased physical activity, or during illness; dose adjustments may be needed. Watch for allergic reactions and signs of low potassium. Rotate sites to reduce lipodystrophy.
Other antidiabetics (additive hypoglycaemia), beta-blockers (may mask hypoglycaemia symptoms), ACE inhibitors/ARBs (may enhance hypoglycaemia), corticosteroids/thyroid hormones/diuretics/sympathomimetics (may raise glucose), alcohol (may increase risk of hypo/hyperglycaemia), MAO inhibitors/salicylates (may enhance hypoglycaemia), thiazolidinediones like pioglitazone (risk of edema/heart failure with insulin)
Store unopened cartridges refrigerated at 2–8°C; do not freeze; keep away from ice pack/freezer compartment. Protect from light; keep in original pack. After first use, store at room temperature (generally below 25–30°C as per pack insert) and use within the recommended in-use period; do not refrigerate in-use pen if label advises. Discard if solution is cloudy or contains particles.
