Cilnidipine is a calcium channel blocker (L/N-type) that relaxes blood vessels and reduces peripheral resistance. Metoprolol is a cardioselective beta-1 blocker that lowers heart rate and cardiac output and reduces renin release, helping lower blood pressure.
Take orally as prescribed, usually once daily at the same time each day. Swallow whole with water; do not crush/chew (metoprolol ER). Can be taken with or after food; continue regularly even if you feel well.
Dizziness, headache, flushing, ankle swelling (edema), fatigue, slow heart rate (bradycardia), low blood pressure, nausea, constipation, cold extremities
Prescription-only. Monitor blood pressure and pulse regularly. May cause dizziness—avoid driving/alcohol until you know the effect. Do not stop metoprolol suddenly (risk of rebound hypertension/angina); taper under medical advice. Use with caution in asthma/COPD, diabetes (may mask hypoglycemia), thyroid disorders, peripheral vascular disease, heart block, bradycardia, heart failure, liver impairment. Seek urgent care for severe breathing difficulty, fainting, very slow pulse, swelling of face/lips, or chest pain.
Other antihypertensives (additive hypotension), diuretics, nitrates, alpha-blockers; verapamil/diltiazem/amiodarone/digoxin (bradycardia/heart block risk); clonidine (rebound hypertension on withdrawal—manage carefully); NSAIDs (may reduce BP control); CYP2D6 inhibitors like fluoxetine/paroxetine/quinidine (increase metoprolol levels); insulin/oral antidiabetics (mask hypoglycemia); alcohol (enhanced dizziness/hypotension).
Store below 25°C in a cool, dry place away from light and moisture. Keep in original blister pack and out of reach of children.
