Type 2 diabetes mellitus (as an adjunct to diet and exercise) to improve glycaemic control; may help reduce risk of hospitalization for heart failure and slow progression of kidney disease in appropriate patients as per physician advice.
Empagliflozin is an SGLT2 inhibitor that reduces glucose reabsorption in the kidneys. This increases urinary glucose excretion, lowering blood sugar, and can also cause mild diuresis leading to reduced blood pressure and weight.
Oral tablet. Usually taken once daily, preferably in the morning, with or without food. Swallow whole with water; take at the same time each day. Dose and duration as prescribed; maintain hydration and follow diet/exercise advice.
Increased urination, thirst, urinary tract infection, genital fungal infection (vulvovaginitis/balanitis), nausea, dizziness, low blood pressure (especially in elderly/diuretics), hypoglycaemia (when used with insulin or sulfonylureas)
Not for type 1 diabetes; risk of diabetic ketoacidosis (may occur even with near-normal sugars)—seek urgent care for nausea/vomiting, abdominal pain, rapid breathing, confusion. Can cause dehydration, low BP and acute kidney injury—use caution in elderly, on diuretics, low fluid intake, or kidney disease. Increased risk of genital infections; maintain hygiene and report symptoms. Temporarily stop before major surgery/acute serious illness as advised. Use caution in severe renal impairment; monitor kidney function. Inform doctor if recurrent UTIs, foot ulcers/infections, or history of ketoacidosis.
Insulin and sulfonylureas (increased hypoglycaemia risk), diuretics like furosemide/thiazides (increased dehydration/low BP risk), antihypertensives (additive BP lowering), lithium (may reduce lithium levels), other SGLT2 inhibitors (avoid duplication)
