Monograph
Biphasic Isophane Insulin (Human) 30/70 injection (30% soluble human insulin + 70% isophane/NPH human insulin), typically 100 IU/mL
Treatment of diabetes mellitus (type 1 and type 2) to control blood glucose; used when a premixed insulin regimen is indicated
This premixed human insulin provides both rapid and intermediate action. The soluble insulin component helps control post-meal glucose spikes, while the NPH (isophane) component provides longer basal control. Together they improve glucose uptake and reduce glucose production by the liver.
For subcutaneous injection only using the supplied pen/cartridge system as advised by the doctor. Usually injected 15–30 minutes before meals; dose and frequency are individualized based on blood glucose. Roll/tilt gently to resuspend until uniformly cloudy (do not shake vigorously); rotate injection sites; monitor glucose regularly.
Hypoglycaemia (low blood sugar),injection site reactions (pain, redness, swelling),weight gain, lipodystrophy at injection site, allergic reactions (rash/itching),oedema
Risk of hypoglycaemia—carry glucose/sugar and know symptoms. Do not use during episodes of hypoglycaemia or if allergic to insulin/excipients. Use caution in kidney or liver impairment, elderly, irregular meals, increased exercise, fever/infection, or after dose changes. Do not inject into a vein; not for insulin pumps. Check label/strength, expiry and appearance; do not use if clumped/grainy or not uniformly cloudy after mixing. Use new needles each time; do not share pens/needles. Seek urgent help for severe hypoglycaemia or allergic reactions.
Other antidiabetics (additive hypoglycaemia),ACE inhibitors and ARBs (may increase hypoglycaemia),beta-blockers (mask hypoglycaemia symptoms),corticosteroids, thiazide diuretics, thyroid hormones, sympathomimetics (may raise glucose),MAO inhibitors (increase insulin effect),alcohol (increases hypoglycaemia risk)
Store unopened in a refrigerator at 2–8°C; do not freeze. Protect from light. Once in use, store as per pack instructions (commonly below 25–30°C) and use within the recommended in-use period; keep cap on and avoid heat/direct sunlight.
