Management of obesity/overweight for weight loss and weight maintenance in adults, as an adjunct to a reduced-calorie diet; helps reduce risk of weight regain and may improve obesity-related risk factors (e.g., dyslipidemia).
Orlistat inhibits gastric and pancreatic lipases in the gut, preventing breakdown and absorption of about 30% of dietary fat. The unabsorbed fat is excreted in stools, leading to reduced calorie intake and weight loss.
Oral capsule. Take 120 mg three times daily with each main meal containing fat, during the meal or up to 1 hour after. Skip the dose if a meal is missed or contains no fat. Follow a reduced-calorie, low-fat diet; take a multivitamin (A, D, E, K) at least 2 hours before/after or at bedtime.
Oily spotting, flatulence with discharge, urgent bowel movements, oily/fatty stools, increased defecation, abdominal pain, diarrhea, fecal incontinence, headache
May cause gastrointestinal fat-related effects, especially with high-fat meals. Rare severe liver injury reported—seek care for jaundice, dark urine, severe abdominal pain, fatigue. Not recommended in chronic malabsorption syndrome or cholestasis. Use with caution in patients with history of kidney stones/oxalate nephropathy; maintain hydration. Monitor INR closely if on warfarin. In diabetes, weight loss may require adjustment of antidiabetic medicines. Not for use in pregnancy; stop if pregnant.
Warfarin/other anticoagulants (may alter INR via vitamin K reduction), Cyclosporine (reduced levels—separate by at least 3 hours and monitor), Levothyroxine (hypothyroidism risk—separate by at least 4 hours and monitor TSH), Antiepileptics such as valproate/lamotrigine (reduced absorption—monitor seizures), Amiodarone (reduced levels), Fat-soluble vitamins and beta-carotene (reduced absorption), Antiretrovirals (reduced exposure—consult specialist)
