Treatment of chronic heart failure (HFrEF) to reduce risk of cardiovascular death and hospitalization; used as a replacement for ACE inhibitor/ARB in suitable patients
Sacubitril inhibits neprilysin, increasing levels of natriuretic peptides that promote vasodilation and diuresis. Valsartan blocks angiotensin II (AT1) receptors, reducing vasoconstriction and aldosterone effects. Together they lower cardiac workload and improve heart failure outcomes.
Oral tablet. Take exactly as prescribed, usually twice daily, with or without food, at the same times each day. Do not take with an ACE inhibitor; allow a 36-hour washout when switching from an ACE inhibitor to sacubitril/valsartan.
Low blood pressure (dizziness),increased potassium (hyperkalemia),kidney function changes,headache,cough,nausea,diarrhea,fatigue
Prescription-only. Risk of angioedema (higher in patients with prior ACEi-related angioedema); stop and seek urgent care if face/lip/tongue swelling or breathing difficulty occurs. Contraindicated in pregnancy; can cause fetal harm. Avoid use with ACE inhibitors (need 36-hour gap) and avoid in patients with history of angioedema related to ACEi/ARB. Use with caution in renal impairment, liver impairment, low blood pressure, dehydration, and in patients with high potassium; periodic monitoring of BP, kidney function and serum potassium is recommended.
