Pantoprazole is a proton pump inhibitor that reduces stomach acid by blocking the H+/K+ ATPase pump in gastric parietal cells. Domperidone is a prokinetic/antiemetic that increases gastric motility and reduces nausea by blocking peripheral dopamine (D2) receptors.
Take as directed by physician. Usually 1 capsule once daily, 30 minutes before breakfast; swallow whole without crushing/chewing (SR/EC).
Headache, diarrhea, abdominal pain, nausea, dry mouth, dizziness, flatulence, constipation
Not for immediate relief of acute heartburn. Use with caution in liver disease. Domperidone may increase risk of QT prolongation/serious heart rhythm problems—avoid in known QT prolongation, significant cardiac disease, electrolyte imbalance, or with other QT-prolonging drugs. Seek medical advice if black stools, vomiting blood, persistent vomiting, unexplained weight loss, or severe abdominal pain occurs. Long-term PPI use may be associated with low magnesium/vitamin B12, fractures, and infections; use lowest effective dose.
Domperidone: strong CYP3A4 inhibitors (ketoconazole, itraconazole, erythromycin, clarithromycin, ritonavir) and other QT-prolonging medicines (amiodarone, sotalol, quinolones, certain antipsychotics) increase arrhythmia risk. Pantoprazole: may reduce absorption of drugs needing acidic pH (ketoconazole, itraconazole, atazanavir); may affect warfarin/INR (monitor), high-dose methotrexate (avoid/withhold PPI if advised).
