Telmisartan is an angiotensin II receptor blocker (ARB). It blocks the action of angiotensin II on AT1 receptors, causing relaxation of blood vessels and reduced aldosterone effects, which lowers blood pressure and reduces strain on the heart.

Tablet for oral use. Take as prescribed, usually once daily at the same time each day, with or without food. Swallow with water; do not stop abruptly without medical advice.

Dizziness, low blood pressure, headache, fatigue, back pain, diarrhea, nausea, increased potassium (hyperkalemia), kidney function changes

Avoid use in pregnancy (can harm fetus). Use with caution in kidney disease, liver impairment, dehydration, low blood pressure, heart failure, renal artery stenosis, and in elderly. Monitor blood pressure, kidney function (serum creatinine) and potassium, especially after starting or dose changes. Seek help if severe dizziness/fainting, swelling of face/lips, or reduced urine output occurs.

ACE inhibitors (e.g., enalapril), aliskiren (contraindicated in diabetes; avoid/monitor), potassium supplements/salt substitutes, potassium-sparing diuretics (spironolactone, amiloride), NSAIDs (ibuprofen, diclofenac) may reduce effect and worsen kidney function, lithium (toxicity risk), diuretics (additive hypotension)
