Treatment of diabetes mellitus (Type 1 and Type 2) to control blood glucose; used when a premixed insulin regimen is advised.
This premixed insulin contains short-acting (regular) insulin for mealtime glucose control and intermediate-acting (NPH) insulin for basal control. It lowers blood glucose by increasing uptake of glucose into muscle and fat and reducing glucose production by the liver.
For subcutaneous use only via compatible insulin pen for cartridges, as prescribed. Usually injected 30 minutes before meals; timing and dose depend on blood glucose, diet and activity. Rotate injection sites (abdomen, thigh, upper arm); do not inject into a vein. Use only if the suspension is uniformly cloudy after gentle rolling; do not shake vigorously.

Risk of severe hypoglycaemia—monitor sugars regularly and carry fast-acting glucose. Dose adjustments may be needed during illness, changes in diet/exercise, renal or hepatic impairment, and when switching insulin brands/strengths (200 IU/mL is a high-strength insulin—avoid dosing errors). Do not use during episodes of hypoglycaemia. Use with caution in patients with recurrent hypoglycaemia or impaired awareness. Not for IV use; cartridge is for pen use and should not be used in syringes. Seek urgent care for allergic reactions.
Other antidiabetics (increased hypoglycaemia), beta-blockers (mask hypoglycaemia symptoms), ACE inhibitors (may increase hypoglycaemia), corticosteroids/thyroid hormones/diuretics/sympathomimetics (may increase glucose), MAO inhibitors and salicylates (may increase hypoglycaemia), alcohol (can cause hypo/hyperglycaemia)
Unopened: store refrigerated at 2–8°C, do not freeze, protect from light. In-use cartridge/pen: keep at room temperature (as per label; commonly below 25–30°C), away from heat/light; do not refrigerate in-use if label advises. Keep out of reach of children.
