Monograph
Zita Pio Met 500 Tablet is a prescription medicine containing Sitagliptin 100 mg + Pioglitazone 15 mg + Metformin 500 mg (as Metformin Hydrochloride). It is used for type 2 diabetes mellitus—adjunct to diet, exercise to improve glycaemic control in adults when single and dual therapy is inadequate. It belongs to the group of medicines called DPP-4 inhibitor + Thiazolidinedione (TZD) + Biguanide.
Tablet for oral use. The dose and how long you take it depend on what you are being treated for and how you respond. Do not take more, or for longer, than your doctor has advised.
The side effects seen most often with Zita Pio Met 500 Tablet are nausea, diarrhoea, abdominal discomfort and headache. Most are mild and settle as the body gets used to the medicine. Tell your doctor if one persists or worries you.
Before you start, tell your doctor about any liver, kidney or heart problem, whether you are pregnant, planning a pregnancy or breastfeeding, and every other medicine you take, including supplements and ayurvedic preparations. Some of them can change how Zita Pio Met 500 Tablet works.
Most side effects of Zita Pio Met are mild and settle as the body gets used to the medicine. The ones reported most often are:
Stop the medicine and get help at once for a rash, swelling of the face or lips, wheezing or difficulty breathing. Tell your doctor about any side effect that persists or worries you.
Tablet for oral use. Take exactly as prescribed, usually with meals to reduce stomach upset. Swallow whole with water; do not crush/chew. Regularly monitor blood glucose/HbA1c as advised.
Metformin decreases hepatic glucose production and improves peripheral insulin sensitivity. Sitagliptin (a DPP-4 inhibitor) increases incretin levels, enhancing glucose-dependent insulin release and reducing glucagon. Pioglitazone (a thiazolidinedione) improves insulin sensitivity in muscle and adipose tissue.
Prescription-only antidiabetic. Metformin: risk of lactic acidosis—avoid in severe renal impairment; assess kidney function before and during therapy; temporarily stop before/after iodinated contrast or major surgery as advised. Pioglitazone: may cause/worsen heart failure due to fluid retention—contraindicated in NYHA class III/IV; monitor for rapid weight gain, oedema, breathlessness; may increase fracture risk; monitor liver enzymes. Sitagliptin: rare pancreatitis/hypersensitivity—seek care for severe abdominal pain or rash. Use caution in elderly, dehydration, liver disease; maintain diet/exercise.
Take it as soon as you remember. If your next dose is almost due, skip the one you missed — never take two doses together to catch up.
An overdose of this medicine may cause low blood sugar, particularly if you also take other diabetes medicines. It may cause low blood sugar and marked stomach upset. It can cause a dangerous build-up of acid in the blood, with vomiting, severe weakness and laboured breathing. Do not wait for symptoms to appear — go to the nearest emergency department and take the pack or strip with you.
Alcohol (increases lactic acidosis risk), iodinated contrast media, insulin/sulfonylureas (increased hypoglycaemia), diuretics/corticosteroids/sympathomimetics (may worsen glycaemic control), gemfibrozil (increases pioglitazone levels), rifampicin (reduces pioglitazone levels), digoxin (levels may increase with pioglitazone), carbonic anhydrase inhibitors like topiramate (may increase acidosis risk with metformin).
Store below 25°C in a dry place. Protect from moisture and light. Keep out of reach of children.
