Type 2 diabetes mellitus (to improve blood sugar control when diet and exercise alone are insufficient); often used when metformin alone is not adequately controlling glucose.

Metformin decreases glucose production in the liver and improves insulin sensitivity, reducing blood glucose levels. Glimepiride stimulates the pancreas to release more insulin, helping lower blood sugar.

Oral tablet. Take exactly as prescribed, usually once or twice daily with food to reduce stomach upset and risk of low blood sugar. Swallow whole with water; do not skip meals while on this medicine.

Nausea, vomiting, diarrhea, abdominal pain, loss of appetite, metallic taste, hypoglycaemia (low blood sugar), dizziness, headache, weight gain

Risk of hypoglycaemia (especially if meals are skipped, with alcohol, or with excess exercise). Metformin may rarely cause lactic acidosis—seek urgent care for rapid breathing, severe weakness, drowsiness, abdominal pain. Use with caution in kidney impairment, liver disease, dehydration, heart failure, elderly patients; dose adjustment/avoidance may be needed. Temporary discontinuation may be required before/after iodinated contrast procedures or major surgery as advised by doctor. Monitor blood glucose and HbA1c regularly.

Alcohol (increases risk of hypoglycaemia and lactic acidosis), iodinated contrast media, insulin/other antidiabetic drugs (additive hypoglycaemia), corticosteroids and thiazide diuretics (may raise glucose), beta-blockers (may mask hypoglycaemia symptoms), ACE inhibitors (may increase hypoglycaemia risk), quinolones, rifampicin, phenytoin, cimetidine

Store below 25°C in a cool, dry place away from moisture and direct light. Keep in original blister pack and out of reach of children.
