What is the FTFT Test?

FTFT stands for Free Thyroid Function Test. It measures free T3, free T4 and TSH. Free hormones are the unbound, active portion in blood, so this panel reflects thyroid status more directly than total T3 and T4 when binding proteins are changed.

High TSH, low free T4
Consistent with an underactive thyroid (primary hypothyroidism).
Low TSH, high free T4 or free T3
Consistent with an overactive thyroid, or a dose of thyroid medicine that is too high.
Normal free hormones, abnormal TSH
Called subclinical thyroid disease. It is usually rechecked, often with thyroid antibodies, before treatment is decided.
