Form 1A is the official medical certificate format used to document fitness-to-drive checkpoints such as vision and hearing, completed by a registered medical practitioner.

Transport/commercial driving licence workflows require Form 1A. Parivahan FAQs also indicate Form 1A is required for applicants aged 40+ in DL services (portal requirement).

For transport categories: yes. For non-transport renewals, Sarathi/Parivahan may ask based on age/category. Follow what your application flow requests and upload accordingly.
