It’s signed by a registered medical practitioner after examining the applicant’s vision, hearing, limb function, and general health, and certifies that the applicant is physically fit to drive. The signed Form 1A is uploaded to Parivahan / Sarathi as part of a driving licence application or renewal. It’s mandatory for renewals after age 40, all transport / commercial licence categories, and whenever the RTO requires medical certification.
These are the six fitness criteria a registered doctor evaluates before signing Form 1A — the same list every RTO and Parivahan upload expects.
Distance and near vision in both eyes (with or without glasses). Colour blindness check is included. Required for both private (LMV) and commercial / transport licences.
Doctor checks whether you can hear normal conversation at 6 metres. A hearing-aid user is fit as long as the aid restores hearing to that standard.
Confirmation that both arms and legs function adequately to control the vehicle, including steering, braking, and shifting gears.
Doctor checks for conditions like uncontrolled epilepsy, severe heart conditions, or any other condition that would make driving unsafe — and notes any present.
Brief check on chronic conditions (diabetes, hypertension), medications, and prior surgeries that could affect driving fitness.
Your name, age, address, and identification photograph — these must match your Parivahan / RTO application exactly.
These get confused often. Form 1 is YOUR self-declaration. Form 1A is a DOCTOR’s certificate.

An RTO-empanelled doctor reviews your case, conducts an online assessment, and signs Form 1A.
The doctor uploads Form 1A directly to your Parivahan / Sarathi application. You receive a confirmation.
Form 1A is only valid when signed by the examining doctor. A self-filled Form 1A PDF — even with a forged signature — gets rejected at the Parivahan upload because empanelled doctors in most states upload Form 1A directly using their Sarathi login, not by accepting a PDF from the applicant. Use the official PDF only to understand the format; the version that counts is the one issued by a registered doctor.
