What the doctor checks on Form 1A

These are the six fitness criteria a registered doctor evaluates before signing Form 1A , the same list every RTO and Parivahan upload expects.

Vision check

Distance and near vision in both eyes (with or without glasses). Colour blindness check is included. Required for both private (LMV) and commercial / transport licences.

Hearing check

Doctor checks whether you can hear normal conversation at 6 metres. A hearing-aid user is fit as long as the aid restores hearing to that standard.

Limb function

Confirmation that both arms and legs function adequately to control the vehicle, including steering, braking, and shifting gears.

Disqualifying conditions

Doctor checks for conditions like uncontrolled epilepsy, severe heart conditions, or any other condition that would make driving unsafe. And notes any present.

General medical history

Brief check on chronic conditions (diabetes, hypertension), medications, and prior surgeries that could affect driving fitness.

Identification

Your name, age, address, and identification photograph. These must match your Parivahan / RTO application exactly.

These get confused often. Form 1 is YOUR self-declaration. Form 1A is a DOCTOR’s certificate.

What is the difference between Form 1 and Form 1A?
Form 1 is a self-declaration: YOU fill it in stating your own medical history (allergies, chronic conditions, medications, etc.). Form 1A is a doctor's certificate: a registered medical practitioner examines you and signs that you are medically fit to drive. Most driving licence applications need Form 1 (always) plus Form 1A (in specific cases. Renewal after age 40, transport / commercial licence, or wherever the RTO requires medical certification).
When do I need Form 1A?
Form 1A is required for: (1) Driving licence renewal after age 40, (2) Transport / commercial vehicle licence applications and renewals, (3) Heavy vehicle (HMV) licence categories, (4) Re-issue of a licence after a long gap, (5) Any case where the RTO specifically asks for medical certification. For a first-time private LMV licence under age 40, only Form 1 is usually needed.

Is there a Form 1A medical certificate PDF I can download?
The official Form 1A format is published as Schedule of the Central Motor Vehicles Rules 1989 and is the same across all states, what changes is the upload process. What are the Form 1A vision requirements?
The doctor checks distance vision in each eye (6/9 corrected for private LMV; 6/6 corrected for transport / commercial), near vision suitable for reading number plates, full visual fields, and colour vision. Glasses or contacts are allowed if they bring your vision up to the required standard. But the certificate will note that you must wear them while driving.
Is Form 1A really mandatory for driving licence renewal after age 40?
Yes. Rule 8 of the Central Motor Vehicles Rules requires medical fitness re-certification at every licence renewal once you cross 40 years of age, regardless of licence class. Some state RTOs are stricter than others about enforcement, but at the Parivahan portal level the medical-certificate field becomes mandatory on renewal applications post-40.
Will the doctor upload Form 1A directly to Parivahan?
