Sacubitril 49 mg + Valsartan 51 mg (as Sacubitril/Valsartan 100 mg film-coated tablet)
Treatment of chronic heart failure with reduced ejection fraction (HFrEF) to reduce risk of cardiovascular death and hospitalization for heart failure.
Sacubitril inhibits neprilysin, increasing levels of natriuretic peptides to promote vasodilation and sodium excretion. Valsartan blocks angiotensin II (AT1) receptors, reducing vasoconstriction and aldosterone effects. Together they lower cardiac workload and improve heart failure outcomes.
Oral tablet. Take exactly as prescribed, usually twice daily, with or without food, at the same times each day. Do not take with an ACE inhibitor; allow a 36-hour washout when switching from an ACE inhibitor to sacubitril/valsartan. Dose may be started lower and titrated based on blood pressure, kidney function and potassium.

Contraindicated in pregnancy; stop immediately if pregnancy occurs. Do not use with ACE inhibitors (need 36-hour washout) or in patients with history of angioedema related to ACEI/ARB. Use caution in low blood pressure, dehydration, kidney disease, renal artery stenosis, hyperkalaemia, severe liver impairment; monitor blood pressure, serum creatinine and potassium. Avoid in diabetic patients taking aliskiren; caution with severe renal impairment. Seek urgent care for swelling of face/lips/tongue or breathing difficulty (angioedema).
