Use with caution in kidney impairment, low platelet count or history of heparin-induced thrombocytopenia (HIT), uncontrolled hypertension, recent surgery, peptic ulcer, or bleeding disorders. Spinal/epidural anesthesia or lumbar puncture can rarely cause spinal hematoma and paralysis—tell your doctor if you have back pain, numbness/weakness or bladder/bowel problems. Regular monitoring may be needed (CBC/platelets, kidney function; anti-Xa in select patients).
