Glimepiride 1 mg + Metformin hydrochloride 500 mg (sustained/modified release)
Type 2 diabetes mellitus (adult) when blood sugar is not adequately controlled with diet and exercise; used as add-on/combination therapy to improve glycaemic control
Glimepiride (a sulfonylurea) increases insulin release from pancreatic beta cells and improves peripheral insulin sensitivity. Metformin reduces glucose production by the liver and decreases intestinal glucose absorption while improving insulin sensitivity.
Take by mouth as prescribed, usually once daily with or just after meals (commonly breakfast) to reduce stomach upset and risk of low sugar. Swallow tablet whole; do not crush/chew if sustained/modified release. Follow diet/exercise plan and monitor blood glucose regularly.

Risk of hypoglycaemia (more likely with missed meals, alcohol, strenuous exercise, elderly, renal/hepatic impairment); carry fast-acting glucose. Metformin can rarely cause lactic acidosis—seek urgent care for rapid breathing, severe weakness, unusual muscle pain, abdominal discomfort, or drowsiness. Use with caution/avoid in severe kidney disease; dose adjustment and periodic renal function monitoring required. Temporarily stop metformin during severe dehydration, serious infection, surgery, or before/after iodinated contrast procedures as advised by doctor. Caution in liver disease, heart failure, and in patients prone to hypoglycaemia. Not for type 1 diabetes or diabetic ketoacidosis.
