Telmevel-H Tablet contains Telmisartan (40mg). It belongs to the Angiotensin II receptor blocker (ARB) + Thiazide diuretic class of medicines.

How Telmevel-H Tablet works

Telmisartan is an angiotensin II receptor blocker (ARB) that relaxes blood vessels and lowers blood pressure. Hydrochlorothiazide is a thiazide diuretic that increases salt and water excretion, reducing blood volume. The combination provides additive blood pressure reduction.

How to take Telmevel-H Tablet

Take by mouth as prescribed, usually once daily at the same time each day, with or without food. Swallow with water; do not stop suddenly without medical advice. If a dose is missed, take it when remembered unless it is close to the next dose.

Take it exactly as your doctor prescribed. Do not change the dose or stop early on your own.

Missed dose and overdose

If you miss a dose

Take it as soon as you remember. If your next dose is almost due, skip the one you missed — never take two doses together to catch up.

If you take too much

An overdose of this medicine can cause a severe fall in blood pressure, with dizziness, fainting and a fast or slow pulse. Do not wait for symptoms to appear — go to the nearest emergency department and take the pack or strip with you.

Most side effects of Telmevel-H are mild and settle as the body adjusts. Reported side effects include:

• electrolyte imbalance (low potassium/low sodium)
• increased uric acid
• increased blood glucose

Warnings and precautions

Not recommended in pregnancy (may harm the fetus), especially 2nd/3rd trimester; avoid if history of angioedema with ARBs. Use with caution in kidney disease, liver impairment, dehydration/low salt states, heart failure, and in the elderly (risk of hypotension). Monitor blood pressure, kidney function (creatinine), and electrolytes (potassium, sodium); hydrochlorothiazide may increase uric acid (gout) and glucose. Avoid excessive sun exposure; hydrochlorothiazide can cause photosensitivity.

Do not take Telmevel-H if

Not for anyone with a known allergy to this medicine or to any ingredient in the pack. Must not be used in pregnancy — it can seriously harm the developing baby. Not for people with narrowing of both kidney arteries, or a history of swelling of the face and throat caused by this group of medicines.

Interactions with other medicines

Potassium supplements/salt substitutes or potassium-sparing diuretics (hyperkalemia risk), NSAIDs (reduced effect and kidney risk), lithium (increased lithium levels/toxicity), other antihypertensives (additive hypotension), ACE inhibitors/aliskiren (renal impairment/hyperkalemia; avoid in diabetes with aliskiren), digoxin (electrolyte changes may increase arrhythmia risk), antidiabetics (may need dose adjustment), corticosteroids/amphotericin B (may worsen hypokalemia)

Store below 25°C in a cool, dry place; protect from moisture and direct light; keep in original blister pack and out of reach of children.

Blood pressure medicines work only while you take them — do not stop because you feel well. Tell your doctor at once if you become pregnant or are planning a pregnancy. Keep a home blood-pressure record if your doctor has asked for one.
