Trajenta Duo 2.5/500 Tablet contains Metformin (1000mg) + Linagliptin (2.5mg). It belongs to the DPP-4 inhibitor + Biguanide class of medicines.

Type 2 diabetes mellitus: to improve glycaemic control in adults when diet and exercise alone are not enough; used when appropriate to treat with both linagliptin and metformin.

How Trajenta Duo 2.5/500 Tablet works

Linagliptin is a DPP-4 inhibitor that increases incretin hormones (GLP-1, GIP), leading to increased insulin release and reduced glucagon in a glucose-dependent manner. Metformin reduces hepatic glucose production and improves peripheral insulin sensitivity, lowering blood glucose.

How to take Trajenta Duo 2.5/500 Tablet

Oral tablet. Take as prescribed, usually twice daily with meals to reduce stomach upset. Swallow whole with water; do not crush/chew. Maintain diet, exercise, and regular blood sugar monitoring.

Take it exactly as your doctor prescribed. Do not change the dose or stop early on your own.

Missed dose and overdose

If you miss a dose

If you miss a dose of Trajenta Duo 2.5mg/1000mg Tablet, take it as soon as possible. However, if it is almost time for your next dose, skip the missed dose and go back to your regular schedule. Do not double the dose.

If you take too much

An overdose of this medicine can cause a dangerous build-up of acid in the blood, with vomiting, severe weakness and laboured breathing. It may cause low blood sugar, particularly if you also take other diabetes medicines. Do not wait for symptoms to appear — go to the nearest emergency department and take the pack or strip with you.

Side effects of Trajenta Duo 2.5/500 Tablet

Most side effects of Trajenta Duo 2.5/500 are mild and settle as the body adjusts. Reported side effects include:

• Nausea
• vomiting
• diarrhoea
• abdominal pain
• loss of appetite
• metallic taste
• headache
• nasopharyngitis
• cough
• hypoglycaemia (more likely with insulin/sulfonylureas)
• vitamin B12 deficiency (long-term)

Stop the medicine and seek help immediately for a rash, swelling of the face or lips, wheezing or difficulty breathing. Tell your doctor about any side effect that persists or worries you.

Warnings and precautions

Risk of lactic acidosis with metformin (rare but serious): seek urgent care for rapid breathing, severe weakness, abdominal pain, drowsiness. Avoid/use caution in kidney impairment; assess eGFR before and during therapy (contraindicated in severe renal impairment). Temporarily stop metformin around iodinated contrast imaging and major surgery as advised. Use caution in liver disease, alcoholism, dehydration, severe infection, hypoxia/heart failure. Pancreatitis has been reported with DPP-4 inhibitors—stop and seek care if severe persistent abdominal pain. Monitor for hypersensitivity reactions and bullous pemphigoid symptoms.

Do not take Trajenta Duo 2.5/500 if

Not for anyone with a known allergy to this medicine or to any ingredient in the pack. Not for people with severe kidney disease, severe liver disease, or an acute illness causing dehydration. Not for people who have had diabetic ketoacidosis. Not for type 1 diabetes or for diabetic ketoacidosis.

Interactions with other medicines

Alcohol (increases lactic acidosis risk), iodinated contrast media, carbonic anhydrase inhibitors (e.g., topiramate, acetazolamide), cationic drugs affecting renal tubular secretion (e.g., cimetidine), insulin and sulfonylureas (increased hypoglycaemia risk), rifampicin and other strong P-gp/CYP3A4 inducers (may reduce linagliptin effect), corticosteroids and diuretics (may raise glucose), beta-blockers (may mask hypoglycaemia symptoms)

Expert advice for Trajenta Duo 2.5/500 Tablet

Trajenta Duo 2.5mg/1000mg Tablet should be taken with food to lower the chances of having an upset stomach. Take it along with regular exercise and a healthy diet to help lower blood sugar levels. Hypoglycemia (low blood sugar level) may occur when taken along with other antidiabetic medicines, alcohol, or by delaying/skipping a meal. Carry a sugar source with you for immediate relief. Monitor your blood sugar level regularly while on treatment with this medicine. Inform your doctor about your diabetes treatment if you are due to have any surgery under a general anesthetic. Tell your doctor immediately if you experience any deep or rapid breathing, persistent nausea, vomiting, or stomach pain, as Trajenta Duo 2.5mg/1000mg Tablet may cause a rare but serious condition called lactic acidosis, which is an excess of lactic acid in the blood.
