Metformin (500mg) + Vildagliptin (50mg)
Type 2 diabetes mellitus (as an adjunct to diet and exercise) when blood sugar is not adequately controlled with metformin alone or other oral antidiabetics; helps control fasting and post‑meal glucose.
Metformin reduces glucose production in the liver and improves insulin sensitivity, lowering insulin resistance. Vildagliptin is a DPP-4 inhibitor that increases incretin hormones (GLP‑1, GIP), leading to increased insulin release and reduced glucagon in a glucose‑dependent manner.
Oral tablet; take exactly as prescribed, usually with meals to reduce stomach upset. Swallow whole with water; do not crush/chew unless advised. Follow diet/exercise plan and monitor blood glucose regularly.

Prescription medicine. Risk of lactic acidosis with metformin (rare but serious): seek urgent care for rapid breathing, severe weakness, muscle pain, unusual sleepiness, abdominal pain. Avoid/use with caution in kidney impairment; dose adjustment or discontinuation may be needed based on eGFR. Use caution in liver disease, dehydration, severe infection, hypoxia, heart failure, and in the elderly. Temporarily stop metformin before/at time of iodinated contrast imaging and during major surgery as advised by doctor. Hypoglycaemia risk increases if used with insulin/sulfonylureas; monitor glucose. Pancreatitis has been reported with DPP‑4 inhibitors—stop and seek care if severe persistent abdominal pain occurs.
