Composition of Ziglim M 2

Ziglim M 2 contains Glimepiride 2 mg + Metformin hydrochloride (SR) 500 mg tablets. It belongs to the Sulfonylurea + Biguanide class of medicines.

How Ziglim M 2 works

Glimepiride is a sulfonylurea that stimulates insulin release from pancreatic beta cells and improves peripheral insulin sensitivity. Metformin reduces hepatic glucose production and improves insulin sensitivity, lowering intestinal glucose absorption. Together they help control fasting and post‑meal blood glucose.

How to take Ziglim M 2

Oral tablet. Take exactly as prescribed, usually once daily with the first main meal (or as advised). Swallow whole; do not crush/chew (SR metformin). Monitor blood glucose regularly; avoid skipping meals to reduce risk of hypoglycaemia.

Take it exactly as your doctor prescribed. Do not change the dose or stop early on your own.

Missed dose and overdose

If you miss a dose

Take it as soon as you remember. If your next dose is almost due, skip the one you missed — never take two doses together to catch up.

If you take too much

An overdose of this medicine can cause severe and prolonged low blood sugar, which may lead to fits or unconsciousness. It can cause a dangerous build-up of acid in the blood, with vomiting, severe weakness and laboured breathing. Do not wait for symptoms to appear — go to the nearest emergency department and take the pack or strip with you.

Side effects of Ziglim M 2

Most side effects of Ziglim M are mild and settle as the body adjusts. Reported side effects include:

• Hypoglycaemia
• nausea
• vomiting
• diarrhoea
• abdominal discomfort
• loss of appetite
• metallic taste
• headache
• dizziness
• weight gain
• skin rash

Stop the medicine and seek help immediately for a rash, swelling of the face or lips, wheezing or difficulty breathing. Tell your doctor about any side effect that persists or worries you.

Safety advice for Ziglim M 2

Warnings and precautions

Risk of hypoglycaemia (especially with missed meals, alcohol, excessive exercise or dose changes). Metformin may rarely cause lactic acidosis—risk increases with severe kidney impairment, dehydration, sepsis, hypoxia, liver disease or heavy alcohol intake. Check renal function before starting and periodically; not recommended in severe renal impairment. Temporarily stop metformin around iodinated contrast studies or major surgery as advised by doctor. Use caution in elderly and in patients with G6PD deficiency (risk of haemolysis) and in those with heart failure or conditions causing poor oxygenation.

Do not take Ziglim M if

Not for anyone with a known allergy to this medicine or to any ingredient in the pack. Not for type 1 diabetes or for diabetic ketoacidosis. Not for people with severe kidney or liver disease. Not for people with severe kidney disease, severe liver disease, or an acute illness causing dehydration. Not for people who have had diabetic ketoacidosis.

Interactions with other medicines

Alcohol (increases risk of hypoglycaemia and lactic acidosis), iodinated contrast media, other antidiabetics/insulin (additive hypoglycaemia), beta-blockers (mask hypoglycaemia symptoms), corticosteroids/diuretics/sympathomimetics (raise glucose), ACE inhibitors (may increase hypoglycaemia), fluoroquinolones (glucose disturbances), azole antifungals and macrolides (may increase sulfonylurea effect), rifampicin/phenytoin (may reduce effect), cimetidine (may increase metformin levels)

How to store Ziglim M 2

Store below 25°C in a dry place, protected from light and moisture. Keep out of reach of children.

Expert advice for Ziglim M 2

Take it shortly before a meal, and eat that meal. Check your sugar as often as your doctor has asked, especially when your routine changes. Take it with or just after food to reduce stomach upset. Keep taking it even when your sugar readings look normal — that is the medicine working. Continue your diet and physical activity; this medicine works alongside them, not instead of them.
