Zoryl M 4 Forte contains Glimepiride (1mg) + Metformin (1000mg). It belongs to the Sulfonylurea + Biguanide class of medicines.

How Zoryl M 4 Forte works

Glimepiride (a sulfonylurea) increases insulin release from the pancreas and improves insulin action in the body. Metformin SR (a biguanide) reduces glucose production by the liver and improves insulin sensitivity, lowering blood glucose without usually causing weight gain.

How to take Zoryl M 4 Forte

Take orally exactly as prescribed, usually once daily with food (preferably with breakfast/main meal). Swallow the sustained-release tablet whole; do not crush or chew. Follow diet/exercise plan and monitor blood glucose regularly.

Take it exactly as your doctor prescribed. Do not change the dose or stop early on your own.

Missed dose and overdose

If you miss a dose

If you miss a dose of Zoryl M 1 Forte Tablet PR, take it as soon as possible. However, if it is almost time for your next dose, skip the missed dose and go back to your regular schedule. Do not double the dose.

If you take too much

An overdose of this medicine can cause severe and prolonged low blood sugar, which may lead to fits or unconsciousness. It can cause a dangerous build-up of acid in the blood, with vomiting, severe weakness and laboured breathing. Do not wait for symptoms to appear — go to the nearest emergency department and take the pack or strip with you.

Side effects of Zoryl M 4 Forte

Most side effects of Zoryl M are mild and settle as the body adjusts. Reported side effects include:

• dizziness
• hypoglycaemia
• weight gain

Stop the medicine and seek help immediately for a rash, swelling of the face or lips, wheezing or difficulty breathing. Tell your doctor about any side effect that persists or worries you.

Warnings and precautions

Risk of hypoglycaemia (more likely with missed meals, excess exercise, alcohol, or with other antidiabetics); carry glucose source. Metformin may rarely cause lactic acidosis—risk increases with severe kidney impairment, dehydration, sepsis, hypoxia, or excess alcohol; kidney function monitoring is required. Use caution in liver disease, elderly, malnourished, and during acute illness/surgery. Withhold metformin before/at iodinated contrast studies and restart only after renal function is stable as advised by doctor.

Do not take Zoryl M if

Not for anyone with a known allergy to this medicine or to any ingredient in the pack. Not for type 1 diabetes or for diabetic ketoacidosis. Not for people with severe kidney or liver disease. Not for people with severe kidney disease, severe liver disease, or an acute illness causing dehydration. Not for people who have had diabetic ketoacidosis.

Interactions with other medicines

Alcohol (increases risk of hypoglycaemia and lactic acidosis), insulin/other antidiabetics (additive hypoglycaemia), beta-blockers (mask hypoglycaemia), corticosteroids/thiazide diuretics/sympathomimetics (raise glucose), ACE inhibitors (may increase hypoglycaemia), NSAIDs and sulfonamides (may increase hypoglycaemia), rifampicin (reduces glimepiride effect), azole antifungals and macrolides (may increase glimepiride effect), iodinated contrast media (lactic acidosis risk with metformin)

Store below 25°C in a dry place, protected from light and moisture. Keep out of reach of children.

Expert advice for Zoryl M 4 Forte

You should continue to exercise regularly, eat a healthy diet, and take your other diabetes medicines along with Zoryl M 1 Forte Tablet PR. Take it with food to lower your chance of having an upset stomach. Monitor your blood sugar level regularly while you are taking this medicine. It can cause hypoglycemia (low blood sugar level) when used with other antidiabetic medicines, alcohol or if you delay or miss a meal. Inform your doctor about your diabetes treatment if you are due to have surgery under a general anesthetic. Tell your doctor immediately if you experience any deep or rapid breathing or if you have persistent nausea, vomiting, and stomach pain as Zoryl M 1 Forte Tablet PR may cause a rare but serious condition called lactic acidosis, which is an excess of lactic acid in the blood. Your doctor may check your liver function regularly. Inform your doctor if you develop symptoms such as abdominal pain, loss of appetite, or yellowing of the eyes or skin (jaundice).
