Metformin hydrochloride 1000 mg (Sustained Release)
Type 2 diabetes mellitus (to control high blood sugar), especially in overweight patients; used alone or with other antidiabetic medicines along with diet and exercise
Metformin reduces glucose production in the liver and decreases absorption of glucose from the intestine. It also improves insulin sensitivity, helping body tissues use glucose better, thereby lowering blood sugar.
Take orally as advised by the doctor, preferably with or after meals to reduce stomach upset. Swallow the sustained-release tablet whole with water; do not crush, chew, or break. Take at the same time each day and follow diet/exercise advice; monitor blood glucose regularly.

Risk of lactic acidosis (rare but serious), especially in kidney impairment, severe dehydration, sepsis, hypoxia, or excessive alcohol intake. Use with caution in liver disease and in elderly patients. Kidney function (eGFR/serum creatinine) should be assessed before and during therapy; dose adjustment/avoidance may be needed in renal impairment. Temporarily stop before iodinated contrast studies and major surgery as advised by doctor. Seek urgent care for symptoms like deep/rapid breathing, severe weakness, muscle pain, unusual sleepiness, abdominal pain, or persistent vomiting.
