Treatment of hypertension (high blood pressure), chronic heart failure (as add-on therapy), and left ventricular dysfunction after myocardial infarction to reduce risk of mortality and hospitalization.
Carvedilol is a non-selective beta-blocker with additional alpha-1 blocking activity. It slows heart rate and reduces the force of contraction (beta-blockade) while also relaxing blood vessels (alpha-1 blockade), lowering blood pressure and reducing cardiac workload.
Oral tablet. Take exactly as prescribed, preferably with food to reduce risk of dizziness/low blood pressure. Swallow with water at the same time each day; do not stop suddenly without medical advice.
Dizziness, fatigue, low blood pressure, slow heart rate (bradycardia), lightheadedness on standing (orthostatic hypotension), headache, nausea, diarrhea, weight gain, edema
Do not stop abruptly (may worsen angina/heart failure). Use with caution in asthma/COPD (may cause bronchospasm), diabetes (may mask hypoglycemia), peripheral vascular disease, thyroid disorders, and in patients with low heart rate or low blood pressure. Contraindicated in severe bradycardia, heart block (unless paced), cardiogenic shock, severe decompensated heart failure requiring IV inotropes, severe liver impairment, and bronchial asthma. Monitor BP, pulse, and signs of worsening heart failure; dose titration should be gradual.
Other antihypertensives (additive hypotension), calcium channel blockers like verapamil/diltiazem (bradycardia/heart block), digoxin (increased digoxin levels and bradycardia), amiodarone/antiarrhythmics (conduction disturbances), insulin/oral antidiabetics (masked hypoglycemia), clonidine (rebound hypertension if stopped incorrectly), NSAIDs (reduced antihypertensive effect), CYP2D6 inhibitors like fluoxetine/paroxetine/quinidine (increased carvedilol levels), rifampicin (reduced carvedilol levels)
