This premix contains 30% short-acting regular human insulin for mealtime glucose control and 70% intermediate-acting NPH insulin for basal control. It lowers blood glucose by increasing peripheral glucose uptake and reducing hepatic glucose production.
Inject subcutaneously only, as prescribed by a doctor (common sites: abdomen, thigh, upper arm; rotate sites). Typically given 30 minutes before meals due to the regular insulin component. Do not use intravenously; resuspend gently (roll) until uniformly cloudy before use; check label/strength and expiry. Dose is individualized based on glucose monitoring.

Risk of severe hypoglycaemia—carry fast-acting glucose and monitor sugars regularly. Do not use if hypoglycaemia is present or if insulin appears clumped/has particles or does not become uniformly cloudy after mixing. Use caution in renal/hepatic impairment, elderly, irregular meals, increased exercise, or during illness; dose adjustments may be needed. Rotation of injection sites reduces lipodystrophy. Educate on recognition and treatment of hypoglycaemia; driving/operating machinery may be impaired during low sugar episodes.
Other antidiabetics (may increase hypoglycaemia), beta-blockers (mask hypoglycaemia symptoms), ACE inhibitors (may enhance hypoglycaemia), corticosteroids (increase glucose), thiazide/loop diuretics (increase glucose), thyroid hormones (increase glucose), sympathomimetics (increase glucose), alcohol (can cause hypo- or hyperglycaemia), MAO inhibitors (increase hypoglycaemia risk), somatostatin analogs and octreotide (alter insulin needs)
Store unopened in a refrigerator at 2–8°C; do not freeze. Keep away from direct heat and light. After opening, store as per pack insert (often refrigerated or at room temperature below ~25–30°C) and discard after the in-use period specified; keep vial/cartridge capped and do not expose to sunlight.
