Treatment of chronic heart failure (especially heart failure with reduced ejection fraction, HFrEF) to reduce risk of cardiovascular death and hospitalization
Sacubitril inhibits neprilysin, increasing levels of natriuretic peptides which promote vasodilation, natriuresis and reduced cardiac remodeling. Valsartan blocks angiotensin II (AT1) receptors, lowering vasoconstriction and aldosterone effects. Together they reduce cardiac workload and improve heart failure outcomes.
Oral tablet. Take exactly as prescribed, usually twice daily, with or without food, at the same time each day. If switching from an ACE inhibitor, maintain a 36-hour washout before starting. Do not stop abruptly without medical advice.
Low blood pressure (hypotension),dizziness,high potassium (hyperkalemia),kidney function changes (increased creatinine),cough,diarrhoea,headache,angioedema (rare but serious)
Contraindicated with concomitant ACE inhibitors (requires 36-hour washout) and in history of angioedema related to ACEi/ARB. Do not use in pregnancy; can harm the fetus. Use caution in low blood pressure, dehydration, kidney impairment, severe liver disease, and diabetes. Monitor blood pressure, serum potassium and renal function periodically; risk of hyperkalemia and worsening renal function. Seek urgent care for swelling of face/lips/tongue or breathing difficulty (angioedema).
ACE inhibitors (contraindicated; 36-hour washout),aliskiren (avoid in diabetes; caution in renal impairment),potassium supplements/salt substitutes and potassium-sparing diuretics (e.g., spironolactone/eplerenone/amiloride),NSAIDs (may worsen kidney function),lithium (may increase lithium levels),other antihypertensives/diuretics (additive hypotension)
