Nitrofurantoin is an antibacterial that is concentrated in urine. It damages bacterial DNA and other essential cellular components after being reduced by bacterial enzymes, leading to bacterial death.
Oral tablet/capsule: take exactly as prescribed, preferably with food or milk to reduce stomach upset. Swallow whole with water; complete the full course even if symptoms improve. Maintain adequate hydration.
Nausea, vomiting, loss of appetite, abdominal pain, diarrhea, headache, dizziness, dark/yellow-brown urine
Use only for lower (bladder) UTIs; not suitable for kidney infection (pyelonephritis) or prostatitis. Avoid/use with caution in kidney impairment (low creatinine clearance) and in elderly with reduced renal function. Contraindicated in G6PD deficiency (risk of hemolysis) and in patients with previous nitrofurantoin-related liver injury. Rare but serious reactions include lung toxicity (acute/chronic cough, breathlessness), hepatitis, peripheral neuropathy, severe skin reactions—stop and seek medical help if these occur. Not recommended at term pregnancy (near delivery) and in neonates.
Antacids containing magnesium trisilicate (reduced absorption), probenecid or sulfinpyrazone (reduced urinary excretion and increased toxicity), uricosuric agents, fluoroquinolones (antagonism reported), agents causing neuropathy (increased risk)
