A health assessment for a named adoption authority and its requested format, with a recorded assessment scope.
What to prepare
Provide the applicant's identity, agency or authority, intended use, required blank format, relevant medical and treatment history, current medicines and available examination or test records. Separate each applicant's assessment where the authority requires individual certificates.
How the assessment works
The clinician reviews the required physical and mental health statements against the information and examination available. Additional assessment or a specified examiner may be needed. Unassessed statements are not automatically marked normal.

Confirm the current agency's requirements before applying. A general fitness certificate is not a substitute for a required authority format or specialist assessment.
