A purpose-specific assessment for a named activity and event, with findings, restrictions and a clinician's conclusion.
What to prepare
Provide the sport, event name and date, distance or level of exertion, organizer's form, previous participation, symptoms during exercise, medical and family history, medicines and relevant records. Explain any prior restriction or recent illness.
How the assessment works
Screening answers guide the clinician's review. A concerning answer requires assessment and may lead to further evidence, tests or an in-person examination. It does not automatically establish fitness or unfitness.
Check the recipient's requirements
Ask the organizer whether they require a particular examination, language, form or examiner qualification. The certificate describes only the activity and scope the clinician assessed.
