Empagliflozin 12.5 mg + Metformin (as Metformin Hydrochloride) 1000 mg
Type 2 diabetes mellitus (to improve glycaemic control in adults) as an adjunct to diet and exercise; may be used when treatment with empagliflozin and metformin is appropriate.
Empagliflozin is an SGLT2 inhibitor that lowers blood glucose by increasing urinary glucose excretion via the kidneys. Metformin decreases hepatic glucose production and improves insulin sensitivity, reducing glucose absorption and improving peripheral uptake.
Oral tablet. Take exactly as prescribed, usually with meals to reduce stomach upset. Swallow whole with water; do not crush/chew. Maintain adequate hydration; follow diet/exercise advice and monitor blood glucose regularly.

Prescription-only antidiabetic. Metformin may rarely cause lactic acidosis—risk increases with kidney impairment, dehydration, severe infection, alcohol excess, liver disease, or hypoxia; stop and seek care if rapid breathing, severe weakness, abdominal pain, or unusual drowsiness occur. Empagliflozin can cause genital/urinary infections, dehydration/low BP, and rare ketoacidosis (can occur even with near-normal sugars); seek urgent care for nausea/vomiting, abdominal pain, breathlessness. Use caution in elderly, on diuretics, or with low blood pressure. Kidney function should be checked before and during therapy; not recommended in severe renal impairment. Temporarily stop around major surgery or serious illness; follow clinician advice. If contrast imaging is planned, metformin may need to be withheld as per medical guidance.
