It can cause a dangerous build-up of acid in the blood, with vomiting, severe weakness and laboured breathing. Risk of hypoglycaemia (especially with missed meals, alcohol, strenuous exercise, elderly, or kidney/liver impairment); lactic acidosis risk with metformin (more likely in severe kidney disease, dehydration, severe infection, shock, alcohol misuse). Contraindicated in severe renal impairment; assess kidney function before and during therapy. Temporarily stop metformin before/after iodinated contrast procedures and major surgery as advised. Use caution in hepatic impairment and in patients with heart failure. Seek urgent care for symptoms of low sugar or lactic acidosis (unusual tiredness, muscle pain, rapid breathing, abdominal pain).
Do not take Glimigrand M2 if
Not for anyone with a known allergy to this medicine or to any ingredient in the pack. Not for type 1 diabetes or for diabetic ketoacidosis. Not for people with severe kidney or liver disease. Not for people with severe kidney disease, severe liver disease, or an acute illness causing dehydration. Not for people who have had diabetic ketoacidosis.
Interactions with other medicines
Alcohol (increases hypoglycaemia and lactic acidosis risk), insulin/other antidiabetics (additive hypoglycaemia), corticosteroids and thiazide diuretics (may raise glucose), beta-blockers (may mask hypoglycaemia symptoms), ACE inhibitors (may enhance glucose-lowering), quinolones (may alter glucose), rifampicin (may reduce sulfonylurea effect), azole antifungals/macrolides (may increase glimepiride effect), iodinated contrast media (increases lactic acidosis risk with metformin)
