Neuropathic (nerve) pain such as diabetic peripheral neuropathy, post-herpetic neuralgia, sciatica/radiculopathy, nerve pain associated with spinal disorders; used when pain has burning, tingling, shooting or numbness features
Pregabalin reduces release of pain-signalling neurotransmitters by binding to the alpha-2-delta subunit of voltage-gated calcium channels in the CNS. Nortriptyline (a tricyclic antidepressant) increases norepinephrine/serotonin activity in pain pathways and modulates pain perception, providing additive relief in neuropathic pain.
Take orally exactly as prescribed, usually once daily at night or as directed; swallow whole with water. Can be taken with or without food; taking at bedtime may reduce daytime drowsiness. Do not stop suddenly—dose should be tapered on medical advice.

May cause dizziness and sleepiness—avoid driving/operating machinery until effects are known. Use caution in elderly, kidney disease (dose adjustment may be needed for pregabalin), glaucoma, prostate enlargement/urinary retention, heart disease/arrhythmias, and history of seizures. Do not stop abruptly (withdrawal symptoms possible). Monitor for mood changes or suicidal thoughts. Risk of additive CNS depression with alcohol/sedatives.
