Type 2 diabetes mellitus: to improve blood sugar control in adults, along with diet and exercise
Sitagliptin is a DPP-4 inhibitor that increases incretin levels, which boosts insulin release and reduces glucagon after meals. Metformin decreases glucose production in the liver and improves insulin sensitivity, helping lower fasting and post-meal blood sugar.
Oral tablet. Usually taken with meals to reduce stomach upset; swallow whole with water. Dose and frequency are as prescribed by the doctor; do not crush or chew. Monitor blood glucose regularly.

Prescription medicine. Risk of lactic acidosis with metformin (higher risk in kidney impairment, dehydration, severe infection, shock, heavy alcohol use). Do not use in severe renal impairment; kidney function should be checked before and during treatment. Temporarily stop metformin before/after iodinated contrast procedures and major surgery as advised. Use with caution in liver disease, heart failure, elderly, and in conditions causing low oxygen. Seek help for symptoms of pancreatitis (severe abdominal pain), allergic reactions, or persistent severe GI symptoms. Monitor for hypoglycaemia when used with insulin or sulfonylureas.
