Glimepiride 3 mg + Metformin hydrochloride (modified/extended release) 500 mg
Type 2 diabetes mellitus (T2DM) in adults when diet and exercise alone or monotherapy is inadequate; helps control blood glucose levels
Glimepiride (a sulfonylurea) increases insulin release from pancreatic beta cells and improves insulin action. Metformin reduces glucose production by the liver and improves insulin sensitivity, decreasing intestinal glucose absorption and improving peripheral uptake.
Oral tablet. Take exactly as prescribed, usually once daily with or immediately after the main meal (often breakfast) to reduce stomach upset and risk of low sugar. Swallow whole (do not crush/chew if modified-release). Monitor blood glucose regularly.

Risk of hypoglycaemia (especially with missed meals, alcohol, heavy exercise, elderly, renal/hepatic impairment); metformin-associated lactic acidosis risk—avoid in severe kidney disease, severe liver disease, hypoxia, sepsis, dehydration. Check kidney function before and during therapy. Temporarily stop metformin for major surgery or iodinated contrast procedures as advised by doctor. Use caution in heart failure, alcohol use disorder, and in patients with vitamin B12 deficiency (long-term metformin).
