Treatment of high blood triglycerides (hypertriglyceridemia) and mixed dyslipidemia; used to reduce risk of pancreatitis in very high triglycerides and as an adjunct to diet and lifestyle changes.
Gemfibrozil is a fibrate that activates PPAR-α, increasing lipoprotein lipase activity and fatty-acid oxidation. This lowers triglycerides and VLDL and can modestly raise HDL cholesterol.
Take orally as directed by a doctor, typically twice daily 30 minutes before morning and evening meals. Swallow capsule whole with water; continue diet/exercise and regular lipid monitoring.
Dyspepsia, abdominal pain, nausea, diarrhea, headache, dizziness, rash, increased liver enzymes
Do not use in severe liver disease, severe kidney disease, or gallbladder disease. Risk of myopathy/rhabdomyolysis is increased when combined with statins (especially simvastatin) or in renal impairment; report muscle pain/weakness. May increase risk of gallstones and pancreatitis; monitor for abdominal pain. Periodic liver function tests and lipid levels are recommended.
Statins (especially simvastatin; avoid), warfarin/other anticoagulants (increased bleeding risk; monitor INR), repaglinide (contraindicated—risk of severe hypoglycemia), CYP2C8 substrates (e.g., pioglitazone; may increase levels), bile acid sequestrants (separate dosing), colchicine (increased myopathy risk)
