Metformin may rarely cause lactic acidosis—risk increases with kidney impairment, dehydration, severe infection, heart/respiratory failure, or excess alcohol; stop and seek care if rapid breathing, severe weakness, or unusual sleepiness occurs. Avoid/use with caution in renal or hepatic impairment; kidney function should be checked before and during therapy. Temporarily stop metformin before iodinated contrast imaging or major surgery as advised by doctor. Not for type 1 diabetes or diabetic ketoacidosis. Use caution while driving if prone to hypoglycaemia.
