Glimepiride 1 mg + Metformin hydrochloride 500 mg (as sustained/extended release)
Type 2 diabetes mellitus in adults when diet and exercise alone or single therapy is insufficient; helps control high blood sugar
Glimepiride (a sulfonylurea) increases insulin release from the pancreas and improves peripheral glucose utilization. Metformin reduces glucose production in the liver and improves insulin sensitivity, lowering blood glucose without usually causing hypoglycaemia when used alone.
Take by mouth as prescribed, usually once daily with the first main meal (or as advised). Swallow whole; do not crush/chew if sustained/extended release. Maintain diet/exercise plan and monitor blood glucose regularly.

Risk of hypoglycaemia (more with missed meals, excessive exercise, alcohol, dose errors); avoid driving/operating machinery if symptoms occur. Use caution in kidney or liver disease; metformin is contraindicated in severe renal impairment and may rarely cause lactic acidosis (seek urgent care for rapid breathing, severe weakness, abdominal pain). Temporarily stop metformin around iodinated contrast studies and major surgery as advised. Monitor renal function, vitamin B12 (long-term), and HbA1c; inform doctor of recurrent low sugars.
