Troycobal Nt contains Methylcobalamin (1500mcg) + Nortriptyline (10mg) + Pregabalin (75mg). It belongs to the B-complex vitamin, Antidepressant and Gabapentinoid class of medicines.
This medicine combines 3 active ingredients: Methylcobalamin, Nortriptyline and Pregabalin. Methylcobalamin replaces a B vitamin your body needs to make red blood cells, to keep nerves healthy and to release energy from food. Nortriptyline restores the balance of the chemical messengers in the brain that regulate mood, sleep and the way pain is felt. Pregabalin calms the over-active nerve signals that cause burning, shooting and tingling pain.
Take this medicine in the dose and duration as advised by your doctor. Swallow it as a whole. Do not chew, crush or break it.
Take it exactly as your doctor prescribed. Do not change the dose or stop early on your own.
Take it as soon as you remember. If your next dose is almost due, skip the one you missed — never take two doses together to catch up.
An overdose of this medicine is unlikely to cause serious harm, because the body passes surplus B vitamins out in the urine. It can cause fits, dangerous heart rhythm disturbance and coma, and is particularly dangerous with the older tricyclic medicines. It can cause deep drowsiness, confusion, unsteadiness and slowed breathing, especially with an opioid or alcohol. Do not wait for symptoms to appear — go to the nearest emergency department and take the pack or strip with you.

Stop the medicine and seek help immediately for a rash, swelling of the face or lips, wheezing or difficulty breathing. Tell your doctor about any side effect that persists or worries you.

Tingling, numbness or burning in the hands and feet should be assessed by a doctor rather than self-treated — a supplement does not address the cause. Folic acid can mask a vitamin B12 deficiency while nerve damage continues, so B12 should be checked before taking folic acid long term. Bright yellow urine after a B-complex dose is expected and harmless. These medicines take two to four weeks to begin working, and longer for the full effect. Stopping at two weeks because nothing has changed is the commonest mistake. Never stop suddenly. Abrupt withdrawal causes dizziness, electric-shock sensations, irritability and flu-like symptoms — your doctor will reduce the dose gradually. Tell your doctor or go to hospital immediately if thoughts of harming yourself appear or worsen, particularly in the first weeks or after a dose change, and especially in anyone under twenty-five. Seek urgent care for agitation with fever, shivering, a racing heart, muscle twitching or confusion, particularly if more than one medicine affecting serotonin is being taken. Side effects are usually worst in the first fortnight and then settle. This medicine causes drowsiness and unsteadiness, particularly when you start it or the dose goes up. Do not drive or operate machinery until you know how it affects you. Never stop it suddenly. Stopping abruptly can cause anxiety, sleeplessness, sweating and, in people taking it for epilepsy, fits. Your doctor will taper it. Tell your doctor about any change in mood or any thoughts of harming yourself. The dose is usually built up slowly from a low starting point — follow the schedule your doctor gave you rather than jumping to the full dose. Pregabalin comes under Schedule H1 from 16 November 2026 and will then need a prescription and a pharmacy register entry.
Not for anyone with a known allergy to this medicine or to any ingredient in the pack. Must not be combined with a monoamine oxidase inhibitor, or started within the gap your doctor specifies after one.
Long-term metformin, acid-reducing medicines and some anti-epileptics lower B12 or folate levels. Tell your doctor if you take levodopa, as vitamin B6 can reduce its effect. Tramadol, triptans for migraine, linezolid, St John's wort and other antidepressants can together raise serotonin dangerously. Anti-inflammatory painkillers and blood thinners increase the bleeding risk when combined with this group. Opioid painkillers, alcohol, sleeping tablets and antihistamines all add to the drowsiness, and the combination with opioids can dangerously slow breathing. Antacids reduce the absorption of gabapentin — separate them by a couple of hours.
Store below 30°C in the original pack, away from direct sunlight and moisture. Keep out of the reach of children.
A supplement corrects a deficiency; it is not a general energy tonic. If you are on it for nerve symptoms, keep your review appointment so the underlying cause is looked for. Give it a fair trial of several weeks, and keep your review appointments. Take it at the same time each day — at night if it makes you drowsy, in the morning if it disturbs sleep. Talking therapy alongside medicine works better than either alone for most people. Take the larger part of the dose at night if drowsiness is a problem. Rise slowly from sitting or lying, especially in the first weeks. Keep your review appointment so the dose can be adjusted to the lowest that controls your pain.
