Tolvaptan 15 mg
Treatment of clinically significant hypervolemic and euvolemic hyponatremia (low sodium) associated with conditions such as heart failure, cirrhosis and SIADH (syndrome of inappropriate antidiuretic hormone secretion), when fluid restriction is ineffective.
Tolvaptan is a selective vasopressin V2-receptor antagonist in the kidney. It blocks the action of antidiuretic hormone (ADH), increasing free water excretion (aquaresis) without significant loss of sodium, thereby raising serum sodium levels and reducing fluid overload.
Tablet to be taken by mouth exactly as prescribed. Commonly started and monitored by a doctor with regular sodium and liver-function checks; take at the same time each day, with or without food. Ensure adequate access to water and drink to thirst; do not restrict fluids unless advised.

Prescription-only medicine. Risk of overly rapid correction of sodium (can cause serious neurological injury); requires close monitoring of serum sodium, especially at initiation and dose changes. Can cause serious liver injury—avoid in patients with underlying significant liver disease unless specifically indicated and monitor LFTs; stop and seek care for fatigue, anorexia, right upper abdominal pain, dark urine or jaundice. Avoid use in patients unable to sense/respond to thirst, in anuria, or needing urgent sodium correction. Use caution in dehydration, hypotension, and with concurrent diuretics. Not for routine use in mild/asymptomatic hyponatremia.
